Community Healthcare System (CHCS) is proud to announce its HomeHealth agency has been named a 2025 HHCAHPS Honors recipient by MatrixCare, a leading provider of CAHPS surveys and analytics for home health agencies. This recognition celebrates agencies that excel in patient satisfaction and quality of care, setting a standard of excellence within the industry.
“The Home Health Honors program reflects the dedication and hard work our team puts into delivering outstanding care,” said Andrea Lutz, Director of Ancillary Services at CHCS. “It inspires us to continually prioritize the patient experience, building trust and compassion in every visit.”
Award eligibility is based on Home Health CAHPS survey results for April 2024 through March 2025. Agencies must have at least one completed survey in each quarter and be contracted with MatrixCare for survey administration. Performance is measured across 19 quality indicators, with agencies scoring above the national benchmark on at least 85% of questions earning Home Health Honors.
“Our nurses, therapists, aides, and support staff work tirelessly to provide compassionate care and support patients in achieving independence at home,” said Kristi Hutfles, RN, Manager of Community HomeHealth. “This recognition is a testament to their dedication and the strong relationships they build with every patient.”
