Outpatient and Home Health Occupational Therapy Services

  • Completes occupational therapy evaluation according to department specific policies, procedures, and standards of care in inpatient, outpatient, skilled nursing, long term care and home health departments.
  • Accurately documents in medical record. 
  • Initiates plan of care with identified patient problems, expected short and long term goals. Assessment reflects individualized patient care needs in inpatient, outpatient, skilled nursing, long term care and home health environments. 
  • Provides high quality patient care as observed/expressed by the patient, family, co-workers and MR audit. 
  • Follows CDC hand hygiene guidelines and Standard Precautions during patient contact and when handling contaminated materials. 
  • Maintains a clean, clutter free work environment including the return of equipment after use. 
  • Provides personal, age appropriate patient-focused care delegating when appropriate to COTA. Continues to monitor and supervise care provided by COTA. 
  • Collaborates with patient, family, physician and other clinical disciplines in decision making regarding each patient's needs. 
  • Maintains/enhances clinical competencies by seeking relevant continuing education opportunities. 
  • Supports and abides by the mission, values, and policies of CHCS in all activities.
  • On-going compliance with CHCS educational requirements.
  • All other duties as required.
