Cardiac Rehab

Cardiac rehab is a medically supervised program for people who have had:

Angioplasty and stenting

Cardiomyopathy

Certain congenital heart diseases

Chest pain (stable angina)

Coronary artery bypass surgery

Coronary artery disease

Heart attack

Heart failure

Heart or lung transplant

Heart valve repair or replacement

Cardiac rehabilitation can help you:

Decrease the risk of coronary artery disease and other heart conditions

Follow heart-healthy behaviors, such as eating healthy and exercising regularly

Improve strength

Learn ways to manage stress and anxiety

Manage weight

Quit bad habits, such as smoking

Referral is required. If you have had any of the conditions above in the past year, talk with your provider to see if cardiac rehab would benefit you. Most insurance companies will pay for all or part of this service.

 

Overall goals of the program are to:

Improve participants' health and quality of life;

Reduce frequency of visits to the hospital or emergency room by helping patients manage their conditions;

Reduce the potential for future heart problems; and

Assist participants in choosing healthier lifestyles.
 

The program helps participants adopt healthy lifestyle changes through exercise counseling and training and education. Coping strategies and support for anxiety are included. Participants are referred to cardio rehab by their primary care provider or cardiologist. If you have questions about referral, contact our Cardiopulmonary Rehab team for assistance. An onsite physician is available for patients’ immediate needs, and our Cardiopulmonary Rehab nurses are trained in advanced cardiac life support.

Heart to Heart Wellness Program (also known as cardiac rehab phase 3)

Heart to Heart is a personalized exercise and education program designed to improve heart health for those who don’t qualify for cardiac rehab phase 2 as well as those who have completed cardiac rehab phase 2 and want to continue their therapy with continued supervision. Those who would benefit include:

  • Those who have completed outpatient phase 2 cardiac rehab. It involves more intensive exercise and activity while you continue to monitor your body's response to increased workloads.
  • Those who have heart disease
  • Those who have risk factors for heart disease which include:
    • High blood pressure
    • High cholesterol
    • Those who are overweight
    • Diabetes
    • Those who are looking to live a healthier lifestyle

Goals of Heart to Heart:

  • Decrease the risk of coronary artery disease and other heart conditions
  • Follow heart-healthy behaviors, such as eating healthy and exercising regularly
  • Improve strength
  • Learn ways to manage stress and anxiety
  • Manage weight
  • Quit bad habits, such as smoking

Supervised Exercise Therapy for Peripheral Artery Disease (SET for PAD)

Supervised exercise therapy is for those with peripheral artery disease with pain in the arms or legs during activity (claudication). The program is a risk factor modification and exercise program to help improve symptoms of claudication (pain, tightness, fatigue in one or more limbs).

The goal of therapy is to extend the time you can be active without claudication. Although not a cure, this therapy can help you reach your maximum level of activity with pain or fatigue and possibly prevent the need for surgery.

Talk with your healthcare provider to see if Supervised Exercise Therapy is right for you. Most insurance companies will cover all or part of this service.
