Rehabilitation Services

Community Rehabilitation offers the expertise of physical, occupational, and speech therapists. Our staff of licensed and certified professionals, together with the support of Community HealthCare System, provide professional, compassionate therapy solutions you can trust. Our therapists implement a progressive, individualized treatment plan that combines patient education, individualized exercise, manual therapy, and a full range of modalities. We work closely with your physician, resulting in a more efficient and effective recovery process. We also provide pediatric speech and occupational therapy at all locations.

Outpatient Services

Outpatient services are available at five specialty clinics that treat specific disorders within the wider range of conditions.


We Offer:

*Physical therapy, which works with you to address neck, back, shoulder and leg pain along with balance and gait impairments.

Physical Therapt

*Occupational therapy, which focuses on treatment of shoulder, elbow, wrist and hand dysfunctions as well as pediatric problems such as poor balance, low muscle tone, excessive distraction, difficulty with how clothes feel, and other issues.

*Speech therapy, which treats cognitive, speech-language and voice disorders as well as swallowing impairments.  

Speech Therapy

Depending on what is needed, treatment options may also include manual therapy, therapeutic exercise and other modalities.

*Pelvic Floor Therapy: Pelvic floor therapy is a specialized service focusing on pelvic function to optimize a person’s quality of life.  The pelvic floor can be injured and experience dysfunction, just like the rest of our body.  Pelvic floor therapists can provide treatments for:

          * Prenatal care

           *Postpartum care

           *Sexual Function (Men and Women)

           *Pelvic Floor Dysfunction (Men and Women)

           *Pain Management (Men and Women)

*Manual therapy, which consists of a variety of hands-on techniques to address bone and muscle pain and dysfunction.

*Therapeutic exercise, including programs customized for each patient using our state-of-the-art rehabilitation equipment.

*Modalities, which include electrical stimulation, iontophoresis, anodyne, and ultrasound to decrease inflammation and pain.

*Mechanical traction: Non-surgical lumbar or cervical spinal decompression helps alleviate tension on the spine by using stretching to decompress the spine. 

*ADLs (Activities of daily living), consists of working on the tasks a person completes in a normal day. ADLs may require specific training in adaptive equipment to increase your ability to perform a task, including: bathing, dressing, toileting, housekeeping, driving and shopping.


Community HealthCare Rehabilitation and Fitness Centers

Rehabilitation and fitness work closely together to offer the best continuity of care. Our rehab and fitness centers are comprehensive, custom facilities that will help you become and stay active. From sports or work injuries to arthritis pain, our centers can treat you to better health.

Our aim is to restore your independence: to get you back in the game, to the farm, or to your family.

Physical, occupational and speech therapy services are also available in our home health program, nursing home, outpatient, and hospital settings.


Physical Therapy

Physical therapy is a personalized assessment and treatment plan that helps individuals improve mobility, control pain, and recover from surgery or injury. It is also beneficial for those who have cardiovascular disease, stroke, Parkinson's, chronic pain, weakness, or a recent surgery.

We work closely with you and your family to create and implement a comprehensive physical therapy treatment plan, whether you are in the hospital (inpatient), in our clinic (outpatient) or in your home.

This plan will address your unique needs and is created by:

Performing a comprehensive (physical, emotional and social) assessment and evaluation of your particular ailment.

Providing a physical therapy diagnosis of your condition and assessing the likely treatment outcome (prognosis).

Implementing a treatment plan, which will include specific exercises, and mechanical treatment.

Providing consultation and determining if and when you need to be seen by another health care professional.

Making recommendations for self-management.

We work in partnership with you, your family and other health professionals to provide family-centered care. You are an important member of our community, and we look forward to working with you.

Our physical therapy service is part of the Department of Rehabilitation Services.


Occupational Therapy

When you have a serious illness or injury, medically based or rehabilitative occupational therapy services may be provided.

Our services improve your health, well-being, functional abilities and independence by emphasizing:

Physical skills to increase movement, strength, and/or coordination

Adaptive, age-appropriate skills necessary to live independently and function safely and appropriately in daily life

Cognitive abilities

Sensorimotor skills

Visual motor and perceptual skills

Social and interpersonal skills

Our occupational therapists will create a treatment plan that addresses your unique needs by:

Collaborating with other health care professionals to identify your needs

Identifying and changing or overcoming barriers that interfere with, restrict or inhibit your functional performance

Teaching you modeling skills and strategies to extend therapeutic interventions

Adapting activities, materials and environmental conditions so you can participate in activities


We work with patients who are staying in the hospital (inpatients) and with people who are not hospitalized (outpatients). We also offer services for children from birth to age 21.

Speech-Language Therapy

Community HealthCare System’s speech-language therapy team provides speech and language evaluations and speech therapy for patients of all ages.

To provide you with the best care possible, our services are:

Based on the most current speech pathology research and best practices.

Provided in a family centered, caring environment.

We treat patients with:

Receptive language delays or disorders – difficulty understanding spoken or written information.

Expressive language delays or disorders – difficulty using language (verbal, non-verbal or written) to communicate with others.

Social-communication disorders, such as autism spectrum disorders.

Cognitive-linguistic deficits, such as memory impairment.

Articulation and phonological disorders – difficulty producing speech sounds correctly.

Motor speech disorders, such as apraxia – difficulty coordinating muscle movements to produce speech.

Voice disorders – problems with the voice, such as hoarseness or loss of voice.

Resonance and airflow disorders, such as speech problems that may occur for children with cleft palate.

Feeding and swallowing disorders.

Fluency disorders, such as stuttering.

Severe communication disorders, who may benefit from augmentative and alternative communication (AAC).
