Cardiopulmonary Rehabilitation

Recovering from cardiac disease or living with chronic lung disease takes strength, determination, and a team of supporters cheering you on every step of the way. You can continue to improve your health right here at Community HealthCare System with our Cardiopulmonary Rehabilitation program. Pulmonary Rehab and Phase 2 Cardiac Rehab are conducted within the Fitness Center at the Onaga campus. We also offer Phase 3 Cardiac Rehab in our Frankfort location. 

Cardiopulmonary Rehab is a medically supervised program for people who have had heart or lung conditions, diseases, or procedures, including heart attack or heart failure, heart valve surgery, coronary artery bypass grafting, angioplasty, COPD (chronic obstructive pulmonary disease), emphysema, interstitial lung disease, chronic bronchitis, COVID-19 or other respiratory infection, or other heart or lung disorders. 

Overall goals of the program are to:

Improve participants' health and quality of life;

Reduce frequency of visits to the hospital or emergency room by helping patients manage their conditions;

Reduce the potential for future heart problems; and

Assist participants in choosing healthier lifestyles.
 

The program helps participants adopt healthy lifestyle changes through exercise counseling and training and education. Coping strategies and support for anxiety are included. Participants are referred to cardiopulmonary rehab by their primary care provider, pulmonologist, or cardiologist. An onsite physician is available for patients’ immediate needs, and our Cardiopulmonary Rehab nurses are trained in advanced cardiac life support.

Heart to Heart Therapy, a flexible program that gives participants access to a team of caring coaches consisting of cardiac nurses, fitness staff, a registered dietitian, and a respiratory therapist, is available in Onaga and Frankfort. The friendly, highly customizable program is designed to help people set goals, learn about heart health risk factors, and start exercising with help from a team of wellness experts. Anyone who is interested can talk to a healthcare provider for a referral. A cardiac nurse will contact referrals to set up an appointment and develop an individualized plan to help participants avoid heart problems.

Some insurance plans may reimburse costs of Heart to Heart Therapy through wellness benefits. Self-pay is also an option. The program costs $60 plus tax each month for four months. All exercise and education sessions as well as extra consultations with staff are included, and nurses also serve as liaisons with physicians and answer questions anytime.

Supervised Exercise Therapy for Peripheral Artery Disease, or SET for PAD, is a program designed for those with symptoms of claudication, which is pain and/or fatigue that occurs with activity. CHCS Cardiac Coaches work with patients who have this problem to extend endurance so they can be active for a longer period of time before experiencing episodes of claudication. SET for PAD can help patients be more active and avoid surgery.
