  • Member of the Long Term Care nursing team, who continuously monitors, evaluates and manages the care given to residents. 
  • Accountable for the development, organization, and implementation of approved policies and procedures as assign and assuming the Director of Nursing position when the DON is unavailable. 
  • Serve as a resource for the staff regarding the resident's cognitive and functional ability.
  • Evaluates the quality and appropriateness of care and the effectiveness and efficiency of systems within the facility setting. 
  • Assist the DON in performance reviews, implementation of nursing service educational programs including, but not limited to, orientation and in-service for licensed and unlicensed personnel. 
  • Assess and evaluate the quality of emotional, mental and physical care being given to long-term care residents.
  • Perform the initial interview and assess new residents.
  • Develop a care plan that has long-term and short-term goals for improvement.
  • Conduct meetings with the multidisciplinary treatment team, caretakers, and families to discuss goals and plans of action.
  • Reach out to specialists within the community to get help in achieving care plan goals.
  • Prepare and electronically transmit timely reports to the national Medicare and Medicaid database.
  • Be an advocate to residents’ rights advocate to ensure that each resident is aware of his or her legal right.
