-Consults with other health care personnel within CHCS, area health care facilities and community agencies to develop continuity of care plan for patients being discharge from the hospital.

-Participates in discharge planning with the patient, patient care team, and patient families.

-Assists with orientation of new associates. Ensures in service and education of Home Health Staff.

-Serves as a resource person and a liaison, coordinating referrals and hospital discharges. Markets services and builds relationships with referring hospitals and agencies.

-Assist and advises in developing and implementing marketing, business and strategic plans for home care program.

-Coordinates the evaluation for medical supplies and equipment required for patient’s home care.

-Participates in recruitment and selection processes for department staff.

-Identifies staff training needs and arranges in-services sessions as needed.

-Complete periodic evaluations of personnel.

-Coaches and counsels staff not meeting performance expectations issuing formal and oral corrective action.

-Attends appropriate CHCS meetings as required

-Performs functions required from a Home Health Nurse.

-Monitor patient satisfaction through patient satisfaction scores and comments; identifies concerns and develops action plan and follow-up

-Identify and plans capital budget and equipment

-Accurately develop annual operating budget within targets.

-Supports and abides by the mission, values, and policies of CHCS in all activities.

-On-going compliance with CHCS educational requirements.

-All other duties as required
