  • Greet visitors to determine what assistance they need and refer to appropriate staff. 
  • Answer phones promptly and direct calls appropriately. 
  • Copy information needed and send in ADR in a timely manner.
  • Input Home Health associates' mileage to Travel Expense Reimbursement spreadsheet and prepare mileage statements for associate reimbursement bi-weekly. 
  • Ensure compliance with the 485's by completing the 485 started by nursing staff, send and track 485's for review and physician signature. 
  • Enter Home Health admitted patients into an excel sheet on public folders and complete excel documentation as required. 
  • Maintain status report on all patients requiring a face-to-face visit and notify Home Health Manager of missing Face to Face documents. 
  • Coordinate and monitor for completion all nursing orders by obtaining physician signature, tracking order for updated status, when returned from physician and file in paper chart. In addition file therapy orders in the paper chart. 
  • Analyze all charts on discharge patients within one week of discharge and notify manager of missing information not received within two weeks of discharge. 
  • Order supplies as needed for all areas of Home Health. 
  • Check eligibility and pre-certify patients whose Medicaid or private insurance require this step. 
  • Call insurance companies for additional visits and update clinicians on approvals and requests from the insurance company. 
  • Print out OBQI reports from the Casper system monthly for the Home Health Manager.
  • Supervise submission and submit as needed all OASIS data to required state system. Review for accuracy and report all concerns/problems to the Home Health Manager for resolution.
  • Complete patient list for billing IDRs for completeness and scan needed forms.
  • Maintain paper documents as needed for patient care.
  • Maintain stock of folders needed for admitting patients to both Community Home Health and Community Based Home Care.
  • Send and maintain information for clients wanting Life Alert Systems.
  • Supports and abides by the mission, values, and policies of CHCS in all activities.
  • On-going compliance with CHCS educational requirements.
  • All other duties as required.
